Welcome to ASSIST

ASSIST is the State of Delaware's Application for Social Service Programs. Here is how ASSIST can help you.

The self-screening questionnaire can tell you which Social Service Programs you and the members of your household may be eligible to receive.
The application lets you apply online for Social Service Programs offered by the State of Delaware.

If you answer the questions in the screening questionnaire, ASSIST can tell you what services the household members may be eligible to receive. The information you give us in the questionnaire is strictly confidential. We do not keep this information. You do not have to fill out the self-screening questionnaire. You can go directly to the online application and apply for all of the services you choose.

If you wish to have someone else manage your case and act as your representative you can tell us in the Additional Information Questions section at the end of this application. Tell us the individual’s name and that you want this person to be your representative.


Note: If you have recently submitted an application or your family is currently receiving benefits please STOP HERE and contact your local DSS office to inquire about the status of your application or your case. The Online Application is for New Applications only!

NOTE: If you are interested in any Long Term Care program AND have a living spouse please skip the screening process. Please complete the on-line application for Long Term Care services or contact your local Long Term Care office.

NOTE: Eligibility for Long Term Care requires that applicants meet medical as well as other (non-financial and financial) eligibility requirements.

NOTE: If you are interested in screening or applying for the Elderly and Disabled Waiver or the Assisted Living Waiver, you should contact the Division of Services for Aging and Adults with Physical Disabilities directly by calling 1-800-223-9074.

NOTE: If you are interested in screening or applying for the Developmental Disabilities Waiver you should contact the Division Developmental Disabilities Service directly by calling 302-369-2180.

NOTE: If you are found eligible for Child Care Services you may be responsible for a parent fee and/or a provider co-pay.

NOTE: If you are going to apply for Child Care Services two additional forms must be printed & signed. Either fax to 302-856-5234 or drop them off at the DSS location nearest you. Click on the following links to open and print:
